Presentation : Chapter 3 - Incident Prevention ( Accident Causation Theories )

Class of DAS1B June 2018 was divided into 3 teams to make a simple presentation regarding Accident Causation Theories each which were Heinrich's Domino Theory, Multi-casuality Accident Model and Bird's Loss Causation Theory. Each group was given about 30 minutes to prepare and 10 minutes to present in front of Puan Syima ( lecturer ) and class.


HEINRICH'S DOMINO THEORY

Group 1 has presented one of the Accident Causation Theories which is the Heinrich's Domino Theory. Below are some of the notes regarding the topic :-


HEINRICH'S DOMINO THEORY  

  • Introduced by Herbert W.Heinrich ( 1930 ) 
  • Injury from an accident is the result of a series of events that is dependent on each other.
  • Injuries are caused by accidents
  •  Accidents are caused by unsafe acts and unsafe condition.
  •  Unsafe acts and unsafe condition are caused by the faults of person.
  •  Faults of person are caused by a personal flaw such as violent temper, nervousness or  ignorance.
  •  The injured worker’ s ancestry and social environment.



MULTI-CASUALITY ACCIDENT MODEL

Group 2 has presented the next Accident Causation Theories which is Multi-Casuality Accident Model. Below are some of the notes regarding the topic :-


MULTI-CASUALITY ACCIDENT MODEL

  • Refers to the facts that there may be more that one cause to any accident.
  • Example: Someone who had tripping accident over a piece of wood lying on a floor in a dark walkway.

BIRS'S LOSS CAUSATION MODEL

Group 3 has presented the last of the Accident Causation Theories which is the Bird's Loss Causation Model. Below are some of the notes regarding the topic :-

BIRD'S LOSS CAUSATION MODEL 

  • Introduced by frank E. Bird ( 1970 )
  • Revised Heinrich Domino.
  • Blaming management
Lack of Management Control
 
     1. Management responsible for: 
  • Selection of workers
  • Machinery and equipment
  • System of work
  • Information and training 
     2. Supervision etc The accident prone worker is a false approach. It is like blaming the victim  instead of the perpetrator


CONCLUSION

ACCEPTED ACCIDENT THEORY

Multiple Causation Theory ✅
A single unsafe act or condition may or may 
not cause an accident but both are caused by 
lack of management control

Bird Loss Causation Model ❌
In line with Schewart (1930’s) theory of quality control

Heinrich’s theory is weak and negative ❌
Blaming victim and lack system thinking, continual improvements, upstream control and worker participation



INTRODUCTION

This blog is about DAS1B, June 2018. This class is under Jabatan Kejuruteraan Awan (JKA), Diploma Alam Sekitar (DAS). This class consist of 34 sttudents, 9 boys and 25 girls. Our Academic advisor (PA) is Puan Nurul Azura Bt Mohd Taib and our Occupational Safety and Health Lecturer (OSH) is Puan Nursyima Nadiah Binti Abu Bakar.
